Submit Purchase Order

Thank you for your order. If you are paying by purchase order, please submit your PO document below so our team can match it to your order and finalize Net 30 invoicing.

What to include

• Your Shopify order number (from your order confirmation email, e.g. #1234)
• Your facility name and the PO number referenced at checkout
• The purchase order document (PDF)
